If I get a game over at any point in the game and I don't have 1000 by the time that game over happens does that mean I have to get 1000 kills from the point I start over after the game over screen?

if you get a game over, you have to start from 0 and get to 1000

I didn't find it too hard, I got it without trying while playing on easy.

If you start a new game, and just play it like normal. You will get it around 2-2, if not before.

If you really want this achievement, get to a place in 1-1 where the enemies continuously spawn from both sides of the screen. Stand on the far left and face right and just spam the fire button. Any enemies that come in on the right will get shot, and any enemies that come in on the left will get your sword. Sometimes a guy will come and kick you over, but it doesn't kill you. I got this in about 10 minutes earlier today, and racked up a couple extra lives as well.

You don't have to get all 1000 kills in one sitting, do you? Can you quit your game and come back later without resetting the kill count?

You can get this achievement with the survive 20 mins on the area 51 lvl. I lasted 20+ mins and I got both achievments ;).
