I joined a random game and the host was on the last level of insane. i attached to him and when we finished the game I got the achievement for beating the game on insane. So I guess you don't need to be all the levels just the final boss.

that sucks, 3 other guys and I played through this game two times today and finished in on normal and insane. The whole way through. At least I did it ligitimately. Not trying to be mean to you at all dude. I would do the same thing. But I still did it ligit.

i joined at the end of a game at normal and then done it all at insane with 3 others its really easy actually only level 6 is any real challenge!

You can only pick the level when you beat the game on hard. Just keep joining random matches with 3 people until you find one that is on the last level. Beat it, get the achievement, and enjoy the level select.
