Yes you have to assasinate one of the primary targets without alerting any guards before hand.

I did it on the doctor just wait for him to walk over to one of the guys on the table then come behind him with hidden blade

You can do it on most of the targets, just make sure the guards don't spot you before you kill the primary target.

I'm pretty sure you can because thats what I've heard from a few other people.

you must assassinate him without alerting any guards and hold down the right trigger which most people do because they run up at him and assassinate him. take out your blade and walk up and kill him.
