I've gotten it as well. Whenever Lucy is around talk to her multiple times even when she says you better get some rest. She has like 2 or 3 responses to tell you to get some rest.
Also the interact message sometimes doesn't show up so make sure you walked around her several times in different directions to make sure you got all the interactions.

Wait - how do you get it? Walk around her hitting the interaction button?
Do I do this just one time until it pops - or every time you're let out of the animus. Since I'm half way through the game, would this require me to start over to get the achievement?

I think I missed the first interaction with her, I didn't know about the achievement at first :(
Guess I'll have to start over just for that.

Is there any way to get this achievement after you've beaten the game? I don't want to go back and re-do every assassination.

You have to restart it if you didn't get it. No other way to do it. Another heads up about something you can't do after it's all over is steal Vidic's pen. Not an achievement task, but something to give you a little more reading to the story arc.
If you had talked to her every time, several times at once occasionally, you would get the achievement after killing the ninth target in Assuf and before the going after the main boss. So technically you won't have to replay the whole game. And if you have been collecting flags and don't want to lose your progress as it overwrites your save, copy it to a memory card, start over and do only the needed amount of missions to proceed (2 for the first three, and 3 for the next six missions) and assassinate your targets. I managed to get it in about 4 or 5 hours. Then delete your new save, and replace it with your old one on the memory card to continue collecting flags.
Just keep talking to her until she says "Aren't you tired?" and "Can't talk gotta work" as that signifies you are done talking this time around.
The first conversation is after memory block 1 when they talk about his past, parents and likeness to Altair. Conversations are always after every memory block, BUT never after waking up in bed. The only thing you can do here is steal Vidic's pen when he's at the window with his back to you if you haven't done it earlier. If you have, then there is nothing to do but get in the Animus.

My friend copied his file to a memory card and speed-played through the whole game again, doing only the bare minimum to beat each city. Took him 5 or 6 hours, which is a bit much for 20 points, but the completist in me will probably do it when I'm done with Mass Effect.
