Some say they are based on a mixture o things.. from my personal experience (keep in mind I now have Bronze/Silve/Gold Star medals) it is Points that dictate it..

Just make sure you go for ALL the GREEN targets before doing the red "TGT" ones.

All you need to do is get an overall "S" rank for each mission, you do not need S rank for every objective like I thought.

I can confirm this. I was browsing through several forums to get this question answered. I read a lot about the S-ranks based on a mixture of time, number of kills etc. This is just not true...you can take as long as you like. Just make sure when you are doing a sub-mission (e.g. operation A - B - C) to select it on your radar. Now every green and red target appears on your radar (the targets from other operations are greyed out and hardly visible on your radar) and you have to kill 'em all. So when you're down to a single red target you have to spare it's life and finish off every remaining green target first. By blazing through operations like this you will score maximum points and get a S-rank every single time. I went through Expert and ACE-mode this way and I got S-ranks on every mission the first time around without even trying. Now I have every SP-achievement and it's about time to work on those mp-ach. :D

Additional suggestion: when you finish an operation you can press the back-button to see if you got a S-rank...if not you can restart (but I never had to since the above layout works perfectly).
